Whether you are deployed yourself or are requesting a package for a deployed loved one, we would be honored to serve you. Complete the form below to request a care package. All submitted information is kept confidential, and we strictly follow OPSEC guidelines.
PLEASE READ THE FOLLOWING BEFORE SUBMITTING REQUEST:
Help us ensure every package reaches the deployed Service Member.
Include an approximate return month so we can mail packages while they are still deployed.
A valid email address is required to verify deployment information when needed.
Notify us immediately if the Service Member returns home early, extends their deployment, or changes mailing addresses by emailing admin@militarymissions.org.
We may occasionally email you to verify deployment information. Requests without a response may be removed from our mailing list.
If submitting several names with the same mailing address and return date, you can list names in the comments section instead of completing separate forms.
